NEW Families complete a Program Request Form for each Kindergarten - 8th grade student attending CMS
RENEWING Families complete a Program Renewal Form for each Kindergarten - 8th grade student attending CMS
Return the following to Renee Ross, Business Manager (rross@cantonmontessori.org)
- Completed Program Request Form
- Birth Certificate
- A recent Utility Bill showing a matching service & mailing address on the bill
Step #2:
Complete the Income Verification Process via the Scholarship Portal
